In preparation for the PA state to come in sometime this year, I was wondering what kind of records they may possibly want for my employees. I have certificates of their training and copies of their BLS cards. I do have an old Dept. of Health Personnel Form. Is that form still required? Also an old Personnel Training and Continued Performance Evaluation Record. Do you have a more current training form? Mine basically lists all laboratory duties, the initial training date and a yearly evaluation date.

Usually once you fill out a personnel form for the State of PA that is all you need to do.In terms of personnel records, make sure you have the following (much of what you had mentioned): 1) qualifications (diploma), 2) signed job description (employee and lab director), 3) training records, 4) annual competency records, 5) annual employee evaluations, 6) continuing education and 7) OSHA records.
